GENERAL PATIENT INFORMATION

TERMS AND CONDITIONS

1. The certificate will have your full name, date of birth, date/time of swab, date of report and most importantly, your results of the swab performed using an RT-PCR assay.
2. Please be aware that up to 1% of PCR tests may need to be re-run daily in order to maintain sensitivity from our laboratories. If it happens to be a rerun, then you will get your results the following day.
3. The Zafash Doctors will not be held liable for losses, costs, damage that you suffer or incur as a result of any delays in receiving test results.
4. If you require Passport numbers to be included on the certificate, then it can be done at no extra cost. Please inform the healthcare professional on arrival for this service.
5. RT-PCR certificate available for travel purposes only. Please note if you test positive then a travel certificate will not be issued.
6. For in clinic appointments please ensure you do not attend your appointment if you have any of the below Covid symptoms.
6.1 A fever (over 37.8oC)
6.2 A new, continuous cough
6.3 Flu like symptoms (runny nose, sore throat, headache, body ache, overwhelming tiredness)
6.4 Shortness of breath
6.5 You have been in contact with someone suspected or known to have coronavirus
6.6 Rashes on the body
7. Please call the clinic and we will reschedule your appointment.
8. Please arrive on time to your appointment and no earlier than 10 minutes prior to your appointment to minimise physical contact with others and you may be subjected to a body temperature check at the entrance of the building
9. .A chaperone will be made available should you require one, please let us know in advance before your appointment.
10. We or our partner laboratories may be legally required to share certain information with Public Health England, or other governmental organisations, in relation to your test, including:
10.1 Personal information (e.g. name and date of birth);
10.2 The result, whether negative or positive; and confirmation that the sample is one that the laboratory has tested for us.
10.3 All positive PCR test results are submitted to Public Health England as part of the National reporting procedures.
10.4 The above information may be used as part of any test and trace service being operated by the government in your jurisdiction.
10.5 By accepting these Conditions of Use you agree to follow the guidance provided by us and our medical partners.
11. We accept no liability for any loss or damage caused by non-compliance with this guidance.
12. By proceeding to book a testing appointment you accept the terms of this privacy notice.
13. Please refrain from eating or drinking half an hour before your appointment.
14. Patients are required to bring ID with their address and Photo for confirming the correct person. This could be either Driving License, ID Card or Passport.
15. We provide a number of different forms of testing (RT-PCR, Antigen).
16. Please note you are solely responsible for determining that the test you are taking is suitable for your particular requirements, including whether any particular test satisfies the standards you are seeking to meet for entry into any particular international destination.
17. Any timescale that Zafash Doctors provide for delivering test results are estimate only, are not guaranteed and no liability for any costs you may incur is accepted e.g. a delayed test result, an inconclusive test result, a false positive/negative result or test results being lost.
18. A positive result indicates that the test has detected the presence of Covid-19 virus.
19. A negative result means that the test has not detected evidence of the Covid-19 virus.
20. Testing is a biological process involving significant complexity and variability, and consequently no testing regime for Covid-19 is 100% accurate in all cases.
21. A negative result does not guarantee that the test subject is free of the Covid-19 virus, nor does a positive result guarantee that the test subject has active, infectious Covid-19.
22. All testing is conducted on this basis.
23. The test results are not a clinical diagnosis. Test results and any comments given by us on the results are for information only. They are not a clinical diagnosis. They are not a replacement for a full and proper medical investigation and/or a doctor’s advice.
24. Our comments are based solely on the results in relation to what are considered normal ranges in the general population. If you have any concerns at all regarding any aspect of your health or your test results you should discuss them directly with your GP or a doctor. Based on the results of your test(s) we may advise that you do a follow-up test or that you see your GP for further investigation or both.
25. You agree that you are solely responsible for acting on such guidance, we accept no liability if you choose not to do so.
26. Payment is by card and cash only upon arrival.
